• You might use Google Forms. 
  Here is an example. Here are some instructions. Anyone with a Google account can create a survey with Google Forms.
• Microsoft have something similar – Microsoft Forms.
  The free version is limited but it is probably good enough. You need access to a Microsoft account. See here for an example. Instructions, Video.
• This is a similar survey using another platform – JISC Online Surveys. It is quite sophisticated, but you need to register through your institution.

• Many institutions have licences for Qualtrics.

• You can also use Survey Monkey, but the free version is rather limited.
